The Congreso Health Center currently operates in person and remotely with a team of bilingual and bicultural medical professionals. We offer comprehensive primary care and family medicine services to the community. In addition, Social Workers and Patient Navigators are available to assist clients with specialty care referrals and health insurance applications for those that qualify.

Eligibility Requirements

There are no eligibility requirements. The Congreso Health Center will serve clients regardless of their insurance status.

Q: How do I enroll as a new patient at Congreso’s Health Center?

A: Patient registration forms need to be completed and dropped off at the front desk.

  • You can also call the PHMC call center to register 267-765-2272.
  • An appointment will be made when you drop it off or you will be called to schedule your first appointment. 

Registration forms are available in English and Spanish (see attachment) 

Q: Can I register regardless of my immigration status?

A: Your immigration status is not a barrier to enrolling or receiving care.

Q: What is the current wait time for a new patient intake appointment (for primary care)?

A: For a new patient the current wait time is 2-4 weeks. Once you are registered as a new patient, you can go to any of the health centers within the PHMC network. 

Q: Are services only provided in English?

A: Congreso’s medical team is bilingual (English/Spanish). If another language is needed the team will use interpreters via phone/telehealth at no cost to you. 

Q: What if I have health insurance?

A: Bring the card and your picture id to every medical visit, co-pay is due at the end of the medical visit, cash is accepted.

  • Call your health insurance company to change your primary care provider to a PHMC provider (ask for instruction form at front desk).
  • Inform the front desk if your insurance changes or you get a new card, also if your address or cell phone number changes.

Q: If I do not have health insurance, what is the cost?

A: The cost of medical visit will be calculated at the end of the visit, and it is due at the end of the medical visit, cash is accepted. If you cannot pay, the balance is due at the next visit or when you receive the bill.  We cannot provide an approximate cost for it depends on the type of visit and what was provided (for example labs, vaccines). Based on your reported income, a sliding fee scale is used to calculate the cost.

A: Congreso’s Health Center is a partnership with PHMC. The medical records are managed by a separate department within PHMC— Please reach out to them directly with any records requests.
